It could have been all sorts of reasons really such as engine failure or downdraughts or many other possibilities. I don't like to ever say pilot error, its not really fair on our fellow pilots, especially when we don't know. Also, when an incident is put down to pilot error, we have to remember that humans make mistakes, and was it actually a lack of training or other human factors that cause highly experienced pilots to have mishaps?
